Praxis Forms

Last updated: 2026-11-02

Create forms and collect structured answers at forms.praxisos.app. Forms also works inside Praxis OS.

Create and share

  1. Choose Create Form and give the form a clear name.
  2. Add fields, choose the appropriate field types, and mark required answers.
  3. Review choice options and the order of questions.
  4. Activate the form when it is ready to collect responses.
  5. Copy its public link and test it as a respondent before distributing it.

Use All Forms to manage forms. Use Response Tables to work with the structured responses associated with them. Changing a form back to draft stops it from actively collecting responses.

Free tier

The free tier includes 3 forms and 100 responses per month, with Praxis branding and iframe embedding. File uploads and submission email notifications are not included.

Pro

Forms Pro is listed at ₦15,000 per workspace per month. Its allowance includes unlimited forms, 2,000 responses per month, uploads up to 25 MB per file within a 2 GB allowance, email notifications, JavaScript embedding, API access, and custom redirects where configured.

Email delivery remains subject to the notification allowance and service availability. Check the product's billing screen for confirmed access before relying on a paid capability.

Embed a form

A public form can be embedded on a website. Configure the allowed website origin for the form. The iframe uses the public form route; the JavaScript embed helper can resize the form and report submission events when enabled by the plan.

Keep secret integration credentials on your server. They should never appear in website JavaScript or a public form.

Protect submissions

Public submissions use origin checks, a honeypot, per-IP rate limits, and Turnstile verification when configured. Uploaded files require a short-lived upload grant bound to the form. Quotas belong to the workspace receiving the responses.

If a submission is refused, review required answers, verification, the form's active status, and its response/upload limits. A saved draft or failed upload is not proof that the server accepted a response.
